ACCIDENT WAIVER AND RELEASE OF LIABILITY

By signing below or electronically agreeing to this waiver, I acknowledge and agree to the following on behalf of myself and/or my child (the “Participant”) regarding participation in the Girls Flag Football Program Prep Camp & 2026 Season Tryouts (hereinafter referred to as the “Activities”) held at Aliso Niguel High School:

Assumption of Risk

I understand that participation in the Activities involves inherent risks, including but not limited to the risk of personal injury, illness, or property damage. I acknowledge that the Activities are conducted in accordance with ANHS Girls Flag Football’s safety policies, including Emergency Action Plans, hydration, and AED procedures, in accordance with district guidelines. I voluntarily assume all such risks, known and unknown, for myself and/or my child as a condition of participation. I acknowledge that without signing this waiver, I and/or my child will not be permitted to participate.

Release and Waiver

On behalf of myself and my child, I hereby waive, release, and discharge the organizers of stated Activities, Aliso Niguel High School, the Capistrano Unified School District, the Activities promoters, sponsors, coaches, volunteers, and any of their directors, officers, employees, agents, representatives, and assigns (“Released Parties”) from any and all claims, demands, causes of action, or liability of any kind—including but not limited to those arising from the negligence or fault of the Released Parties—related to any injury, illness, disability, death, or property damage that may occur during or as a result of participation in the Activities.

Fitness to Participate

I certify that both myself and my child are physically fit, have sufficiently prepared for participation in the Activities, and have no health-related conditions that would prevent safe involvement. I have not been advised otherwise by a qualified medical professional.

Medical Treatment

I consent to receive (and to have my child receive) any medical treatment deemed advisable in the event of injury, accident, or illness during the Activities. I agree that any personal medical or accident insurance for myself and/or my child shall be the insurance of first resort before any coverage provided by the Released Parties.

Photography & Media Release

I understand that photos, videos, or other media may be taken during the Activities. I grant permission for my and/or my child’s likeness to be used in promotional, educational, or informational materials by Aliso Niguel High School, the Capistrano Unified School District, and their assigns, without compensation.

Tryouts/Roster Selection Acknowledgement

I understand that participation in the ANHS Program Prep Camp, clinics, workouts, or 2026 Season Tryouts does not guarantee a spot on the 2026 ANHS Girls Flag Football roster. I acknowledge that roster selections are made solely at the discretion of the coaching staff based on evaluations that may include, but are not limited to, skill level, athletic ability, effort, attitude, coachability, positional needs, team fit, attendance, and overall program standards. I understand that team needs and roster limitations may impact final selections. By participating, I acknowledge that all decisions made by the coaching staff regarding team placement and roster selection are final.

Indemnification

I agree to defend, indemnify, and hold harmless the Released Parties from and against any and all claims, losses, damages, liabilities, or expenses (including reasonable legal fees, or similar costs) arising out of or related to participation by myself and/or my child in the Activities.

Entire Agreement

This Waiver and Release constitutes the sole, complete, and binding agreement regarding the release and indemnification of liability for participation in the Activities. It supersedes any prior oral or written agreements. No modification of this document shall be valid unless made in writing and signed by all parties.

BY SIGNING OR ELECTRONICALLY AGREEING TO THIS WAIVER, I ACKNOWLEDGE THAT I HAVE READ THIS DOCUMENT, UNDERSTAND ITS CONTENTS, AND I VOLUNTARILY AGREE TO ITS TERMS. I AM AWARE THAT BY AGREEING TO THIS, I AM WAIVING CERTAIN LEGAL RIGHTS FOR MYSELF AND/OR MY MINOR CHILD, AND I DO SO VOLUNTARILY.
