ACCIDENT WAIVER AND RELEASE OF LIABILITY

 

EVENT: Girls Flag Football Camp

• Day 2:  Tuesday, December 30 | 1:00 to 3:00 PM

• Day 3:  Wednesday, December 31 | 1:00 to 3:00 PM

LOCATION: Aliso Niguel High School, Aliso Viejo, CA

OPEN TO:  3rd–8th Grade Girls

By signing below, I acknowledge and agree to the following on behalf of myself and/or my child (the “Participant”) regarding participation in the Girls Flag Football Camp (hereinafter referred to as the “Activity”) held at Aliso Niguel High School:

Assumption of Risk

I understand that participation in the Activity involves inherent risks, including but not limited to the risk of personal injury, illness, or property damage. I voluntarily assume all such risks, known and unknown, for myself and/or my child as a condition of participation. I acknowledge that without signing this waiver, I and/or my child will not be permitted to participate.

Release and Waiver

On behalf of myself and my child, I hereby waive, release, and discharge the Activity organizers, Aliso Niguel High School, the Capistrano Unified School District, the Activity promoters, sponsors, coaches, volunteers, and any of their directors, officers, employees, agents, representatives, and assigns (“Released Parties”) from any and all claims, demands, causes of action, or liability of any kind—including but not limited to those arising from the negligence or fault of the Released Parties—related to any injury, illness, disability, death, or property damage that may occur during or as a result of participation in the Activity.

Fitness to Participate

I certify that both myself and my child are physically fit, have sufficiently prepared for participation in the Activity, and have no health-related conditions that would prevent safe involvement. I have not been advised otherwise by a qualified medical professional.

 

Medical Treatment

I consent to receive (and to have my child receive) any medical treatment deemed advisable in the event of injury, accident, or illness during the Activity. I agree that any personal medical or accident insurance for myself and/or my child shall be the insurance of first resort before any coverage provided by the Released Parties.

Photography & Media Release

I understand that photos, videos, or other media may be taken during the Activity. I grant permission for my and/or my child’s likeness to be used in promotional, educational, or informational materials by Aliso Niguel High School, the Capistrano Unified School District, and their assigns, without compensation.

Indemnification

I agree to defend, indemnify, and hold harmless the Released Parties from and against any and all claims, losses, damages, liabilities, or expenses (including reasonable legal fees, or similar costs) arising out of or related to participation by myself and/or my child in the Activity.

 

Entire Agreement

This Waiver and Release constitutes the sole, complete, and binding agreement regarding the release and indemnification of liability for participation in the Activity. It supersedes any prior oral or written agreements. No modification of this document shall be valid unless made in writing and signed by all parties.

 

 

 

I CERTIFY THAT I HAVE READ THIS DOCUMENT, UNDERSTAND ITS CONTENTS, AND AGREE TO ITS TERMS. I AM AWARE THAT BY SIGNING BELOW I AM WAIVING CERTAIN LEGAL RIGHTS FOR MYSELF AND/OR MY CHILD, AND I DO SO VOLUNTARILY.
